RizenRx — Telehealth Services Disclosure and Consent Information

Last Updated: October 30, 2026

1. Introduction

RizenRx provides an online platform that facilitates access to telehealth services through independent, appropriately licensed healthcare professionals.

This page explains how telehealth services work through RizenRx, including the potential benefits, risks, limitations, patient rights, and responsibilities associated with receiving healthcare services remotely.

RizenRx is a brand and online platform operated by GH Services LLC.

GH Services LLC operates the RizenRx website and coordinates certain administrative, customer service, payment, and order-related activities. It is not itself a medical practice or healthcare provider and does not independently diagnose medical conditions or prescribe medications.

Medical evaluations, treatment recommendations, and prescription decisions are made by independent healthcare professionals.

Please review this information before requesting or receiving telehealth services through RizenRx.

2. What Is Telehealth?

Telehealth involves delivering healthcare services through electronic communications, information technology, or other permitted methods when the patient and healthcare professional are not physically located in the same place.

Telehealth may be used for medical evaluation, diagnosis, treatment recommendations, follow-up care, and patient education.

Depending on your medical needs and the services determined appropriate by your healthcare professional, telehealth may involve:

Online medical intake questionnaires and review of submitted health information.

Electronic transmission of medical records, photographs, and other relevant health information.

Secure messaging and asynchronous communications.

Real-time communication through video, audio, or telephone when appropriate and available.

Electronic follow-up communications regarding treatment recommendations and prescriptions.

Not every consultation requires a live video or telephone appointment. A healthcare professional may review submitted medical information asynchronously and determine whether additional communication or evaluation is necessary.

3. RizenRx Healthcare Provider Network

RizenRx works with LocumTele as its healthcare provider network partner to facilitate access to independent, appropriately licensed healthcare professionals.

3.1 Patient Intake Process

Patients begin by completing an online medical intake questionnaire and providing the health information requested.

The information submitted is used to facilitate review by an available clinician from the LocumTele provider network.

3.2 Rotating Clinician Model

After you submit your medical intake form, an available clinician may be assigned to review your information.

RizenRx operates with a rotating clinician model. This means:

The clinician assigned to your consultation may vary.

Different clinicians may review separate consultations or treatment requests.

Your assigned clinician may depend on availability, scheduling, and applicable state licensing requirements.

You are not guaranteed the same clinician for every consultation or follow-up request.

The clinician assigned to your care is responsible for independently reviewing your medical information and determining whether treatment is clinically appropriate.

3.3 Clinician Identity and Communication

The identity and professional credentials of the clinician assigned to review your information will be made available through the applicable patient communication or account system.

You will also be provided with an appropriate method of contacting the clinician responsible for your care.

You may contact your assigned clinician with questions regarding your medical evaluation, treatment recommendations, prescription instructions, or follow-up care.

3.4 Independent Medical Judgment

All medical evaluations, diagnoses, treatment recommendations, and prescription decisions are made independently by the assigned healthcare professional.

The clinician may:

Approve or decline a treatment request.

Request additional medical information.

Request further communication or evaluation.

Recommend alternative treatment.

Determine that an in-person examination is necessary.

Decline to issue or renew a prescription.

Submitting a medical intake form does not guarantee that a prescription or particular treatment will be provided.

GH Services LLC and RizenRx do not independently practice medicine or make clinical prescribing decisions.

4. Geographic Availability and Licensing

RizenRx accepts medical intake requests from individuals located in all 50 United States.

Medical evaluations and prescription services are provided only where permitted by applicable federal and state laws and regulations.

The healthcare professional responsible for your consultation must be appropriately licensed or otherwise authorized to provide care in the state where you are physically located when the healthcare service is provided.

Medication availability and pharmacy fulfillment may also vary depending on your location and applicable pharmacy requirements.

Submitting an intake form does not guarantee that a consultation, prescription, medication, or shipment will be available in your state.

5. Potential Benefits of Telehealth

Telehealth may provide several benefits, including:

Convenient access to healthcare services without requiring an initial in-person office visit.

Reduced travel time and related expenses.

The ability to submit medical information electronically.

Greater flexibility when requesting medical evaluations and treatment.

Opportunities to communicate with healthcare professionals remotely.

More efficient coordination of certain follow-up services.

Telehealth may be appropriate for certain conditions and treatment requests, but it is not suitable for every patient or medical situation.

No particular medical result or treatment outcome is guaranteed.

6. Potential Risks and Limitations of Telehealth

Telehealth has potential risks and limitations, including the following.

6.1 Limitations of Remote Evaluation

Information provided to your healthcare professional may be incomplete, inaccurate, or insufficient to support an appropriate medical decision.

Your clinician may be unable to conduct certain physical examinations, diagnostic procedures, laboratory testing, or other assessments that may be available during an in-person visit.

The inability to perform an in-person examination may limit the clinician's ability to identify or evaluate certain medical conditions.

6.2 Technology and Communication Risks

Technical difficulties, internet interruptions, software problems, or other technology failures may delay or prevent communication with your healthcare professional.

Electronic information may be delayed, transmitted incorrectly, or temporarily unavailable.

6.3 Privacy and Security Risks

Although reasonable safeguards are used to protect personal and health information, electronic communications and data transmission are not completely risk-free.

Unauthorized access, security incidents, or data breaches may occur despite reasonable precautions.

6.4 Treatment Limitations

Your healthcare professional may determine that your medical condition cannot be appropriately evaluated or treated through telehealth.

You may be asked to provide additional information, undergo testing, attend an in-person examination, or seek care from another healthcare professional.

6.5 Prescription Limitations

Federal and state laws, provider licensing requirements, professional standards, and pharmacy regulations may restrict which medications can be prescribed or dispensed through telehealth.

Your clinician independently determines whether a prescription is clinically appropriate and legally permitted.

A prescription may be declined even if you have previously received the same medication or treatment from another healthcare professional.

6.6 Delays or Unavailable Services

Provider availability, technical difficulties, pharmacy operating limitations, regulatory restrictions, and other circumstances may delay or prevent requested services.

RizenRx does not guarantee that every consultation, prescription, medication, or shipment will be available within a particular period.

7. Alternatives to Telehealth

Telehealth is voluntary.

You may decline telehealth services and seek an in-person evaluation from a healthcare professional of your choice.

You may also ask your assigned clinician whether an in-person examination or another treatment approach is more appropriate for your condition.

Your healthcare professional may recommend an in-person visit when remote evaluation is not sufficient to provide appropriate care.

Declining telehealth may mean that RizenRx cannot provide the particular healthcare service requested through its online platform.

8. Medical Emergencies

RizenRx is not an emergency medical service.

Telehealth services through RizenRx are not intended to provide emergency diagnosis, emergency treatment, or continuous medical monitoring.

If you believe you are experiencing a medical emergency, call 911 or seek immediate medical attention at the nearest emergency department.

Do not delay emergency treatment while waiting for a response from RizenRx, LocumTele, your assigned clinician, or a dispensing pharmacy.

Do not use ordinary email, text messages, or the RizenRx website to report an emergency.

9. Privacy and Confidentiality

Personal and health information submitted through RizenRx is handled in accordance with the RizenRx Privacy Policy and applicable federal and state privacy laws.

Depending on the services requested, relevant information may be shared with:

LocumTele and its applicable provider-network systems to facilitate clinician assignment.

The clinician assigned to review your medical information.

The medical practice or healthcare provider entity responsible for your care.

GH Services LLC and authorized service providers when necessary to operate the platform and coordinate administrative services.

The dispensing pharmacy when a prescription is authorized and pharmacy fulfillment is required.

If your clinician authorizes a prescription, relevant information may be transmitted to the pharmacy assigned to fulfill your prescription.

RizenRx currently coordinates prescription fulfillment through:

The pharmacy assigned to your prescription may vary depending on the medication, your location, applicable requirements, and pharmacy availability.

Each pharmacy operates independently and may maintain its own pharmacy and prescription records under applicable privacy and pharmacy laws.

9.2 Medical Records and Privacy Notices

The healthcare professional or medical provider entity responsible for your care may maintain its own medical records and provide a separate Notice of Privacy Practices when required by law.

Dispensing pharmacies may also provide their own privacy notices.

Please review the RizenRx Privacy Policy and any applicable healthcare provider or pharmacy privacy notices for additional information regarding the collection, use, disclosure, and retention of your information.

10. Patient Access to Medical Information

Medical records created or maintained by your healthcare professional or medical provider entity are subject to applicable recordkeeping, confidentiality, access, and retention requirements.

You may request access to your medical information in accordance with applicable law and the procedures established by the healthcare provider responsible for maintaining those records.

You may also request prescription records from the pharmacy responsible for filling your prescription.

For administrative assistance in identifying the appropriate healthcare provider or pharmacy, you may contact RizenRx at support@rizenrx.com.

11. Voluntary Participation and Withdrawal of Consent

Your participation in telehealth is voluntary.

You may decline telehealth services before receiving care or withdraw your consent to telehealth at any time by notifying your assigned healthcare professional.

You may also contact RizenRx at:

Email: support@rizenrx.com

Withdrawal of consent may affect the availability of services through the RizenRx platform. You may instead seek in-person healthcare services from a provider of your choice.

Withdrawal of consent does not automatically require deletion of medical records, prescription records, billing records, or other information that must be retained under applicable law.

Where required by applicable law, your healthcare professional or medical provider entity will obtain and document your consent before providing telehealth services.

Consent may be obtained through an appropriate written, electronic, or verbal process, depending on the applicable state requirements and the healthcare provider's procedures.

12. How Telehealth Consent Is Documented

This page provides general information about telehealth services offered through RizenRx. Viewing this page or accessing the Privacy Policy does not, by itself, constitute or document your informed consent to receive telehealth services.

Before telehealth services are provided, the healthcare professional or responsible medical provider entity will obtain and document consent using a process permitted by applicable law.

Depending on the applicable requirements, consent may be documented through an electronic or written acknowledgment, a verbal consent recorded in the medical record, or another legally permitted method.

The responsible healthcare provider is responsible for maintaining appropriate documentation of telehealth consent in accordance with applicable professional and state requirements.
