RizenRx — Terms of Service

Last Updated: October 30, 2026

Please read these Terms of Service (“Terms”) carefully before accessing or using the RizenRx website, products, or services. These Terms constitute a legally binding agreement between you and GH Services LLC, the company operating the RizenRx website and brand.

By accessing the website, creating an account, submitting a medical intake form, purchasing a product, or using any service offered through RizenRx, you acknowledge that you have read, understood, and agree to these Terms.

If you do not agree with these Terms, you should not use the website or services.

1. Company Information

RizenRx is a brand and online platform operated by GH Services LLC (“Company,” “we,” “us,” or “our”).

Business Information

GH Services LLC
5710 Tonopah Drive
San Jose, CA 95123
Email: support@rizenrx.com
Website: https://rizenrx.com

RizenRx provides an online platform through which customers may submit medical intake information, access telehealth services provided by independent healthcare professionals, and purchase prescription products when authorized.

GH Services LLC operates the RizenRx website and coordinates certain administrative, customer service, payment, and order-related activities.

GH Services LLC is not itself a medical practice, healthcare provider, or dispensing pharmacy. It does not independently diagnose medical conditions, prescribe medications, compound medications, dispense prescriptions, or provide pharmacist counseling.

2. Eligibility

You must be at least 18 years old to use RizenRx, submit a medical intake form, request prescription treatment, create an account, or purchase prescription products.

By using the website, you represent and warrant that:

You are at least 18 years old.

You are legally capable of entering into a binding agreement.

The personal, contact, and medical information you provide is accurate and complete.

You are accessing the services from a location where the applicable services may legally be provided.

RizenRx does not knowingly provide prescription services to individuals under 18 years of age.

We reserve the right to refuse service, restrict access, or cancel an order if eligibility requirements are not satisfied.

3. Description of Services and Business Relationships

RizenRx facilitates access to online healthcare-related services and prescription products through relationships with independent healthcare professionals and licensed dispensing pharmacies.

Depending on the service or product requested, the process may involve:

RizenRx providing the website, customer support, administrative services, intake process, and order coordination.

LocumTele facilitating access to an available healthcare professional from its provider network.

The assigned healthcare professional independently reviewing the patient's submitted medical information and determining whether treatment is clinically appropriate.

An authorized dispensing pharmacy receiving a valid prescription, processing the prescription, and dispensing and shipping the medication when appropriate.

GH Services LLC may collect payment from customers for charges displayed during checkout, including applicable product, consultation, and service charges.

GH Services LLC does not make independent clinical decisions and does not control the professional judgment of healthcare providers or the dispensing decisions of independent pharmacies.

Healthcare professionals and pharmacies involved in an order operate independently and retain responsibility for their respective professional, clinical, pharmacy, and regulatory obligations.

4. Geographic Availability and Nationwide Services

RizenRx accepts medical intake requests from individuals located in all 50 United States.

The availability of medical evaluations, prescriptions, specific medications, and pharmacy fulfillment is subject to applicable federal and state laws and regulations.

Medical evaluations and prescriptions are provided only when an appropriately licensed or otherwise authorized healthcare professional is available to provide care in the state where the patient is physically located at the time of the consultation or evaluation.

Prescription medications are dispensed and shipped only when the dispensing pharmacy is authorized to fulfill the prescription in the applicable state.

Treatment eligibility, medication availability, pharmacy assignment, and fulfillment may vary depending on state requirements, provider availability, pharmacy authorization, and product-specific restrictions.

Submitting an intake form or placing an order does not guarantee that a medical evaluation, prescription, medication, or shipment will be provided.

5. Telehealth Services and LocumTele Provider Network

RizenRx works with LocumTele as its healthcare provider network partner to facilitate access to independent, appropriately licensed healthcare professionals.

LocumTele facilitates access to clinicians who may review patient intake information and provide medical services through the applicable telehealth process.

GH Services LLC and RizenRx do not independently practice medicine or make clinical prescribing decisions.

5.1 Patient Intake Process

Patients begin the process by completing an online medical intake questionnaire and providing the health information requested.

Patients are responsible for submitting truthful, accurate, and complete medical information, including information about current medications, allergies, medical conditions, and other information relevant to the requested treatment.

Incomplete or inaccurate information may affect the clinician's ability to evaluate a treatment request.

5.2 Assignment of Healthcare Professionals

After the patient submits the intake form, an available clinician from the LocumTele provider network may be assigned to review the patient's information.

The assigned clinician may vary based on availability, state licensing requirements, provider scheduling, and other applicable considerations.

RizenRx operates with a rotating clinician model. Patients are not guaranteed that the same clinician will review every consultation, follow-up request, or subsequent treatment request.

The assigned healthcare professional is responsible for independently reviewing the information submitted by the patient and determining whether additional information, an evaluation, or another form of medical interaction is necessary.

5.3 Clinician Identity and Credentials

Before medical care is provided, patients will be informed of the identity and professional credentials of the clinician assigned to review their medical information.

Information identifying the assigned clinician, including applicable professional credentials, will be made available through the applicable patient communication or account system.

Patients will also be provided with an appropriate method of contacting the clinician responsible for their care.

Because RizenRx uses a rotating clinician model, the assigned clinician may differ between consultations or treatment requests.

5.4 Independent Clinical Decisions

All medical evaluations, diagnoses, treatment recommendations, and prescription decisions are made independently by the assigned licensed healthcare professional.

A clinician may:

Approve or decline a treatment request.

Request additional medical information.

Request further evaluation or consultation.

Recommend an alternative treatment.

Determine that a particular medication is not clinically appropriate.

Decline to issue or renew a prescription.

Submitting an intake form does not guarantee a consultation, prescription, medication, treatment outcome, or prescription renewal.

5.5 Patient Communication With Clinicians

Patients may contact the healthcare professional responsible for their care through the communication method provided for their consultation.

Patients may contact their clinician regarding medical questions, treatment recommendations, prescription instructions, follow-up concerns, or other matters related to their care.

Patients may also contact the dispensing pharmacy directly for medication-specific questions, pharmacist counseling, prescription status, and pharmacy-related concerns.

6. Prescription Products and Pharmacy Fulfillment

Prescription medications offered through RizenRx require evaluation by an appropriately licensed healthcare professional and a valid prescription where required.

A prescription will only be issued when the clinician independently determines that the requested treatment is clinically appropriate and legally permitted.

RizenRx does not guarantee that every medication, strength, dosage form, or formulation will be available in every state.

6.1 Pharmacy Partners

RizenRx coordinates prescription fulfillment through independent pharmacy partners. Depending on the medication, patient's location, applicable state requirements, pharmacy authorization, and product availability, a prescription may be assigned to one of the following pharmacies.

Rush Pharmacy

1977 Alafaya Trail, Suite 1121
Oviedo, FL 32765
Telephone: 407-542-0880
Fax: 866-913-8207
Website: https://rushpharmacy.com/

Epiq Scripts LLC

465 W President George Bush Highway, Suite 240
Richardson, TX 75080
Telephone: 833-654-3553
Fax: 888-522-6059
Website: https://epiqscripts.com/

The pharmacy responsible for dispensing a particular prescription will be identified through the applicable order information, pharmacy communications, or prescription documentation.

Not every pharmacy partner necessarily fulfills every medication or ships to every destination.

6.2 Independent Pharmacy Responsibilities

Each dispensing pharmacy operates independently and is responsible for its own pharmacy operations, prescription processing, pharmacist review, medication dispensing, labeling, packaging, shipping, pharmacist counseling, and applicable regulatory obligations.

Patients may contact the dispensing pharmacy directly for questions about their medication, prescription, refills, shipping, adverse reactions, and pharmacist counseling.

A pharmacy may decline to fill a prescription when required by law, professional judgment, its policies, or operational limitations.

GH Services LLC and RizenRx are not dispensing pharmacies and do not independently compound or dispense prescription medications.

7. Compounded Medications and Product Disclosures

Certain prescription medications offered through RizenRx may be compounded medications.

Compounded medications are not FDA-approved as finished drug products. The FDA does not review compounded medications for safety, effectiveness, and quality before they are marketed in the same manner as FDA-approved drug products.

The fact that an ingredient may be used in an FDA-approved medication does not mean that a compounded preparation containing that ingredient is FDA-approved.

Compounded medications are not the same as FDA-approved generic drugs.

RizenRx does not represent compounded medications as:

FDA-approved finished drug products.

FDA-approved generic medications.

Equivalent to a particular FDA-approved brand-name medication.

Clinically proven to produce the same results as an FDA-approved medication.

Approved, reviewed, or certified by the FDA when that is not the case.

Where a product is compounded, its compounded status will be disclosed on the applicable product page and in the purchasing process.

Compounded medications may be provided only when permitted by applicable law, based on the independent clinical judgment of the prescribing healthcare professional and the requirements applicable to the dispensing pharmacy.

Product descriptions, ingredients, strengths, formulations, directions, and other medication information should be reviewed carefully before use.

Always follow the prescription label and instructions provided by your healthcare professional and dispensing pharmacy.

8. Medical Information and Disclaimer

Information provided on the RizenRx website is for general informational and educational purposes only.

Website content is not a substitute for professional medical advice, diagnosis, or treatment.

You should not disregard professional medical advice or delay seeking medical care because of information found on the website.

Always seek the advice of a qualified healthcare professional regarding a medical condition, medication, symptom, treatment, or other healthcare concern.

RizenRx does not guarantee any particular medical result, treatment outcome, or response to medication.

Individual treatment decisions are based on each patient's medical history, information provided, clinical evaluation, and the independent judgment of the healthcare professional.

9. Medical Emergencies

RizenRx is not an emergency medical service.

Do not use the website, online intake process, or telehealth services for medical emergencies.

If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency medical attention.

If you experience a serious or unexpected reaction after taking medication, seek appropriate medical assistance immediately and contact your healthcare professional and dispensing pharmacy as appropriate.

10. Account Registration and Security

Certain RizenRx services may require you to create an account.

You agree to provide accurate, current, and complete information when registering and to update your information when it changes.

You are responsible for:

Maintaining the confidentiality of your account login credentials.

Securing access to your account and devices.

All activity conducted through your account.

Notifying RizenRx promptly if you suspect unauthorized account access.

You may not create an account using false information, another person's identity, or another person's account without authorization.

We reserve the right to suspend or restrict accounts that appear to be compromised, fraudulent, or used in violation of these Terms.

11. Medical Intake Information

By submitting a medical intake form, you acknowledge that the information will be used to facilitate review by an appropriately assigned healthcare professional.

You agree that:

Your responses are truthful and complete to the best of your knowledge.

You will disclose relevant medical conditions, allergies, medications, and other requested health information.

You will update information when it changes.

You will respond to reasonable requests for additional information.

You understand that incomplete or inaccurate information may affect treatment decisions.

You understand that submitting an intake form does not itself establish that a prescription will be issued.

A clinician may determine that additional information, follow-up communication, or an alternative form of care is necessary.

12. Orders, Pricing, and Payment Authorization

By submitting an order, you authorize GH Services LLC and its designated payment processors to charge your selected payment method for the amounts displayed during checkout.

Depending on the order, charges may include:

Prescription product or medication charges.

Medical consultation or healthcare service charges.

Shipping and handling charges.

Applicable taxes.

Subscription or recurring charges, where selected.

The applicable prices, fees, and recurring charges will be disclosed during checkout.

You agree to provide accurate payment information and maintain a valid payment method for transactions.

We reserve the right to correct pricing or listing errors, refuse or cancel orders, limit quantities, or request additional information when reasonably necessary.

If an order is canceled after payment has been collected, any refund or payment reversal will be handled in accordance with these Terms and applicable law.

13. Subscription and Recurring Billing

Certain RizenRx products or services may be offered through a subscription or recurring-order program.

Where available, the applicable subscription frequency, price, billing amount, and renewal terms will be displayed before enrollment.

By enrolling in a subscription, you authorize recurring charges to your selected payment method according to the terms disclosed during checkout.

13.1 Automatic Renewal

Unless canceled, your subscription will continue according to the renewal schedule disclosed at enrollment.

Your selected payment method may be charged automatically on each renewal date for the amount disclosed during enrollment, subject to any changes communicated in advance as required by law.

13.2 Subscription Cancellation

You may request cancellation by:

Logging into your RizenRx account and using available subscription-management options; or

Contacting support@rizenrx.com.

To avoid the next recurring charge, submit your cancellation request before the next scheduled billing date.

Cancellation of a subscription does not necessarily cancel an order that has already been submitted for clinical review, prescription processing, fulfillment, or shipment.

You should review your account or contact customer support if you need confirmation of cancellation.

13.3 Subscription Changes

We may update subscription prices, product availability, or subscription terms.

Material changes to recurring charges will be communicated before they take effect when required by applicable law.

14. Order Cancellation, Refunds, and Returns

14.1 Prescription Medication Returns

For patient safety, prescription medications that have been dispensed or shipped generally cannot be returned for resale or redistribution.

Do not send prescription medication back to RizenRx or a dispensing pharmacy without first receiving instructions from the pharmacy.

Unused or unwanted medication should be disposed of in accordance with applicable pharmacy guidance and local medication-disposal requirements.

14.2 Refund Requests

Prescription medication charges may become non-refundable once a prescription has been dispensed or shipped, subject to applicable law.

However, customers may contact RizenRx regarding:

Duplicate or incorrect charges.

Orders canceled before fulfillment.

Lost or damaged shipments.

Incorrect products or fulfillment issues.

Billing or payment-processing errors.

Other concerns relating to an order.

Refund requests will be reviewed based on the circumstances, stage of fulfillment, applicable pharmacy requirements, and applicable law.

Any consultation or service fees that may be non-refundable after a consultation or service has been performed will be disclosed before payment.

Nothing in these Terms eliminates consumer rights or remedies that cannot legally be waived.

14.3 Non-Prescription Products

If non-prescription products are offered, returns and refunds will be handled according to the applicable product-specific terms displayed at purchase and applicable law.

15. Shipping and Delivery

Prescription medications are shipped by the dispensing pharmacy after a valid prescription has been received and processed.

Shipping methods, carriers, and delivery estimates may vary depending on the dispensing pharmacy, product, destination, and handling requirements.

Where available, shipment tracking information may be provided through email or the customer's account.

Delivery estimates are not guarantees.

RizenRx is not responsible for carrier delays, weather-related disruptions, incorrect delivery information, regulatory restrictions, or other circumstances beyond its reasonable control.

Customers are responsible for providing a complete and accurate shipping address and for notifying RizenRx of changes before an order is submitted for fulfillment.

If a package is lost, damaged, or appears to have been delivered incorrectly, contact RizenRx support and, where applicable, the dispensing pharmacy so the matter can be reviewed.

16. Communications and Electronic Notices

By creating an account or placing an order, you agree that RizenRx may send service-related communications, including:

Account notices.

Order confirmations.

Medical intake updates.

Prescription-related administrative notices.

Shipping updates.

Billing notices.

Customer service communications.

Communications may be sent by email, telephone, text message, or through the applicable account or patient communication system.

Message and data rates may apply to text messages.

You may opt out of marketing emails through the unsubscribe link included in the email.

Where applicable, you may opt out of marketing text messages by replying STOP.

Consent to receive marketing communications is not a condition of purchasing products or receiving healthcare services.

You agree to maintain accurate contact information and take reasonable steps to protect the privacy of communications received through your email account, telephone, or mobile device.

17. Privacy and Health Information

Your use of RizenRx is also governed by our Privacy Policy, which describes how information submitted through the website may be collected, used, processed, stored, and shared.

To coordinate services, certain information may be shared with independent healthcare professionals, LocumTele or its applicable provider-network partners, dispensing pharmacies, payment processors, and other service providers as permitted by applicable law and described in the Privacy Policy.

Healthcare professionals and dispensing pharmacies may maintain their own medical, prescription, and health information records and may provide separate privacy notices or Notices of Privacy Practices.

Patients should review applicable privacy notices provided by the healthcare professional and dispensing pharmacy involved in their care.

Information submitted through the medical intake process may include sensitive medical and health information. You should submit such information only through the designated intake or patient communication systems intended for that purpose.

18. Acceptable Use

You agree to use RizenRx only for lawful purposes and in accordance with these Terms.

You may not:

Provide false, misleading, or fraudulent information.

Impersonate another person or misrepresent your identity.

Use the website to engage in unlawful activity.

Attempt to access another person's account or private information without authorization.

Introduce malware, viruses, or other harmful code.

Interfere with website operations or security.

Use unauthorized automated systems to scrape or extract website content.

Distribute spam or unsolicited commercial messages through the website.

Infringe the intellectual property or privacy rights of another person.

Purchase prescription products for unlawful resale or distribution.

You may communicate directly with your assigned healthcare professional and dispensing pharmacy regarding your care, prescriptions, and medication.

We reserve the right to investigate suspected violations and suspend or terminate access when appropriate.

19. Website Content and Intellectual Property

Unless otherwise stated, the RizenRx website, including its text, graphics, design, logos, photographs, software, and other materials, is owned by or licensed to GH Services LLC.

You may access and use website content for personal, non-commercial purposes.

You may not reproduce, distribute, modify, publish, sell, or commercially exploit website materials without prior written permission.

RizenRx names, marks, and logos may not be used without authorization.

20. Third-Party Services and Websites

The RizenRx website may contain links to third-party websites or make available services provided by independent healthcare professionals, provider networks, pharmacies, payment processors, and other third parties.

These third parties operate independently and may have separate terms, privacy policies, and professional obligations.

RizenRx does not control all third-party websites or services and is not responsible for their independent content, availability, or operations.

Your interactions with third parties may be subject to separate agreements and applicable law.

Nothing in these Terms changes the professional obligations of an independent healthcare professional or dispensing pharmacy.

21. Disclaimer of Warranties

To the fullest extent permitted by applicable law, the RizenRx website and its administrative services are provided on an “AS IS” and “AS AVAILABLE” basis.

GH Services LLC does not guarantee that the website will always be available, uninterrupted, secure, or error-free.

To the extent permitted by law, we disclaim implied warranties, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement.

This section does not modify the professional obligations of healthcare providers or pharmacies and does not waive any rights or warranties that cannot legally be waived.

22. Limitation of Liability

To the fullest extent permitted by applicable law, GH Services LLC will not be liable for indirect, incidental, special, punitive, or consequential damages arising out of or relating to your use of the website or its administrative services.

GH Services LLC is not responsible for independent clinical decisions made by healthcare professionals or the independent pharmacy operations of dispensing pharmacies.

Nothing in these Terms excludes or limits liability for matters that cannot legally be excluded or limited, including liability arising from fraud, willful misconduct, or other non-waivable conduct.

To the extent permitted by applicable law, GH Services LLC's total liability for claims arising from the website or its administrative services will not exceed the greater of:

The amount you paid directly to GH Services LLC for the relevant service during the twelve months preceding the event giving rise to the claim; or

One hundred U.S. dollars ($100).

23. Indemnification

To the extent permitted by applicable law, you agree to indemnify and hold harmless GH Services LLC, its officers, employees, and representatives from third-party claims, liabilities, damages, and reasonable expenses arising directly from your unlawful use of the website, fraud, or material violation of these Terms.

This obligation does not apply to claims caused by our own negligence, willful misconduct, or conduct for which indemnification is prohibited by law.

24. Suspension and Termination

We may suspend or terminate your access to the website or account if:

You materially violate these Terms.

We reasonably suspect fraud or unauthorized account activity.

Continued access would create a legal, security, or operational risk.

We are required to do so by law or a regulatory authority.

You may request account deactivation by contacting support@rizenrx.com.

Termination of your RizenRx account does not automatically delete medical or prescription records maintained by independent healthcare professionals or pharmacies.

Those records may be retained in accordance with applicable laws and the record-retention policies of the healthcare professional or pharmacy responsible for them.

Any provisions that by their nature should survive termination will remain in effect.

25. Dispute Resolution

If you have a concern or dispute regarding RizenRx, please first contact us at support@rizenrx.com and provide a description of the issue so that we may attempt to resolve it informally.

Nothing in these Terms prevents you from contacting a relevant licensing board, consumer protection agency, healthcare regulator, government authority, healthcare professional, or dispensing pharmacy.

Nothing in this section limits rights or remedies that cannot legally be waived.

26. Governing Law

These Terms will be interpreted and enforced in accordance with applicable federal and state laws.

The governing law and venue for any dispute will be determined under applicable conflict-of-law rules and mandatory consumer protection requirements.

Nothing in this section deprives you of legal protections that cannot be waived under the laws applicable to you.

28. Changes to These Terms

We may update these Terms periodically to reflect changes in our services, business operations, legal requirements, or other circumstances.

The updated version will be posted on the RizenRx website with a revised “Last Updated” date.

Changes will become effective upon posting unless otherwise stated or required by law.

Your continued use of the website after the effective date of revised Terms constitutes acceptance of the updated Terms to the extent permitted by applicable law.

29. General Provisions

If any provision of these Terms is found to be invalid or unenforceable, the remaining provisions will remain in effect to the fullest extent permitted by law.

Our failure to enforce any provision does not constitute a waiver of that provision.

You may not transfer or assign your rights under these Terms without our prior written consent.

We may assign these Terms in connection with a merger, reorganization, sale, or transfer of business assets, subject to applicable law.

These Terms, together with the applicable policies and agreements referenced herein, govern your use of the RizenRx website and services.
